LPOA for Non-Brokerage Accounts form

Please use this form to authorize a Limited Power of Attorney (“LPOA”) on account(s) held at a mutual fund, annuity, insurance, or 529 company (“Account(s)”). The form must  be signed and dated by the client. It stays in house here at Mutual, but serves as our permission from the client to allow you to transact on their behalf. Mutual recommends having this form on file with us for every client account. 

Some of the requests you would be able to make are:
  1. address changes
  2. one time withdrawals (529 and mutual fund companies only)
  3. one time contributions (529 and mutual fund companies only)
  4. decrease, increase or cancel systematic withdrawals or contributions (529 and mutual fund companies only)
  5. trading and rebalancing portfolio requests 

Helpful Tips:
  1. The form allows for multiple accounts to be listed at once. Please make sure all information is filled out.
  2. If you are listing a joint or trust account with multiple owners/trustees, then all parties must sign a LPOA each giving us permission to accept instructions from your firm. 
  3. We accept the LPOA form signed via DocuSign with the Certification of Completion pages included. 
  4.  This form must have your firm's name and advisor's name(s) under the "Authorization to Add Limited Power of Attorney" section
