Corporate/Non-Corporate Resolution Authorization Form

For any Corporation, Partnership, Sole Proprietorship, Non-Profit, and LLC. accounts we will need the Corporate/Non-Corporate Resolution Authorization Form completed. This Form is used to name the individuals who are authorized to act on behalf of the business organization such as transactions for the account. Please submit along with the Non-Brokerage Account Application and the Change of Broker Dealer Form. 

Provide the Name of the Company, the Account Number where the account is held, and the Type of Organization:


Have the Secretary or a Managing Member of the Company fill out the Authorizing Resolution section:


Have the Corporate Officers / Designees that can act on behalf of the Company sign the form:


Have the Secretary or Managing Member sign the form:


Lastly, imprint the Company's Corporate Seal on the form, OR provide a copy of the companies Articles of Incorporation along with the form.
